A severe impact on quality of life is caused by OA, which causes joint pain, stiffness, edema, and diminished function.<\/span><\/p>\n<blockquote><p><span data-preserver-spaces=\"true\">In Ayurveda, this condition can be correlated to Sandhigata Vata, where aggravated Vata Dosha accumulates in the joints, leading to pain, stiffness, crepitus(cracking sound), and reduced mobility. The cartilage degeneration is said to be caused by Dhatu Kshaya (tissue depletion), namely Asthi (bone) and Majja (bone marrow) Dhathu.<\/span><\/p><\/blockquote>\n<h4><span data-preserver-spaces=\"true\">LAKSHANAS OF SANDHIGATA VATA(SYMPTOMS):<\/span><\/h4>\n<ul>\n<li><span data-preserver-spaces=\"true\">Sandhi shoola(joint pain): Aching pain that worsens with movement of joints.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Shotha(swelling): Mild or moderate swelling in the joints affected.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Sparshana Asahatwa(Tenderness): pain while touching the joint.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Prasarana-Akunjana Pravritti Vedana(pain during movement): Flexion and extension becomes difficult.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Atopa(crepitus\/crackling sound): During the movement of a joint, a sound is heard.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Hanti Sandhi(Joint stiffness and restricted movement): It is more common in the morning and after prolonged rest.<\/span><\/li>\n<\/ul>\n<h4><span data-preserver-spaces=\"true\">ETIOLOGY OF OSTEOARTHRITIS:<\/span><\/h4>\n<p><span data-preserver-spaces=\"true\">The etiology of OA involves multiple factors.<\/span><\/p>\n<h5><span data-preserver-spaces=\"true\">1. Age-related changes:<\/span><\/h5>\n<p><span data-preserver-spaces=\"true\">Aging is a primary risk factor for OA:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Since the chondrocyte function diminishes, it reduces the ability to synthesize and maintain the extracellular matrix.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Increased cartilage softening and erosion due to proteoglycan and collagen degradation.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Increased joint stress steers to subchondral bone stiffening.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Impairment in lubrication due to reduced synovial fluid viscosity.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">2. Mechanical Factors:<\/span><\/h5>\n<p><span data-preserver-spaces=\"true\"> Joint stress can accelerate cartilage wear and tear, leading to OA.<\/span><\/p>\n<p><span data-preserver-spaces=\"true\">Contributing factors\u00a0 include:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Obesity: Increases the strain on weight joints (hips, knees), and with adipokines,\u00a0 inflammation is heightened.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Repetitive joint use: Overuse in jobs (such as construction workers), sports, and hobbies that require a lot of kneeling or squatting.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Joint misalignment: Uneven joint loading and cartilage wear occur due to Genu Varum or Valgum.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">3. Genetic and Familial Factors.<\/span><\/h5>\n<p><span data-preserver-spaces=\"true\">Genetics also plays a role in OA:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Hand and Knee OA: Mostly seen in women.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Collagen mutations: Early onset of\u00a0 OA occurs due to mutations in COL2A1(Type II collagen gene).\u00a0<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Cartilage metabolism and bone remodeling are affected by variations in BMP(Bone Morphogenetic Protein).\u00a0<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">4. Metabolic and Endocrine Disorders:<\/span><\/h5>\n<p><span data-preserver-spaces=\"true\">The risk of OA increases due to systemic inflammation and altered bone\/cartilage metabolism by certain metabolic disorders.<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">\u0627\u0644\u0633\u0643\u0631\u0649<\/span><span data-preserver-spaces=\"true\">: Chondrocyte apoptosis and cartilage stiffness occur due to Advanced glycation end products(AGEs).<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Hyperlipidemia<\/span><span data-preserver-spaces=\"true\">: Causes oxidative stress and joint inflammation.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Hemochromatosis<\/span><span data-preserver-spaces=\"true\">: Excess iron deposition leads to cartilage degeneration.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Acromegaly<\/span><span data-preserver-spaces=\"true\">: Joint hypertrophy and degeneration due to excess growth hormone.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">5. Inflammatory and Biochemical Factors:<\/span><\/h5>\n<p><span data-preserver-spaces=\"true\">Low-grade inflammation is a crucial part of OA, even though it is mainly a non-inflammatory type of arthritis.<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Cytokines(IL-1, TNF-alpha): Stimulates matrix metalloproteinases(MMPs), leading to cartilage breakdown.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Leptin and Adipokines(released from adipose tissue): Promote inflammation and cartilage degradation.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Oxidative stress: Leads to chondrocyte apoptosis and reduction in cartilage repair capacity.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">6. Post-Traumatic and Secondary OA:<\/span><\/h5>\n<p><span data-preserver-spaces=\"true\">OA can develop due to previous joint injuries or underlying diseases:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Joint injuries(Post Traumatic OA): Fractures, ligament tears(like ACL injury), and meniscal damage accelerate cartilage wear.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Inflammatory arthritis: Rheumatoid arthritis(RA), gout, and septic arthritis can lead to secondary OA.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">A lack of blood flow causes avascular necrosis, which results in bone collapse and cartilage destruction.<\/span><\/li>\n<\/ul>\n<h1><span data-preserver-spaces=\"true\">CLINICAL FEATURES OF OSTEOARTHRITIS(OA):<\/span><\/h1>\n<h5><span data-preserver-spaces=\"true\">1. Pain:<\/span><\/h5>\n<p><span data-preserver-spaces=\"true\">The common symptom of OA.<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">It increases with activity and improves with rest as it is mechanical.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">At first, the pain is intermittent, but as the illness worsens, it becomes persistent, even when sleeping or at rest.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">The morning stiffness normally subsides after 30 minutes (unlike rheumatoid arthritis, where it lasts longer).<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">In severe situations, pain may radiate to nearby locations, such as hip pain radiating to the thigh or knee pain spreading to the shin.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">2. Stiffness:<\/span><\/h5>\n<ul>\n<li><span data-preserver-spaces=\"true\">Morning stiffness occurs but usually less than 30 minutes.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">The gelling phenomenon where stiffness occurs after periods of inactivity,(eg: after sitting for long periods, movement is initially stiff but improves gradually).<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">3. Crepitus:<\/span><\/h5>\n<ul>\n<li><span data-preserver-spaces=\"true\">During the joint movement, a grating or crackling sound is heard or felt.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">It occurs due to the rubbing of roughened cartilage surfaces.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Commonly seen in the knee joint.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">4. Swelling:<\/span><\/h5>\n<ul>\n<li><span data-preserver-spaces=\"true\">Due to synovial inflammation, mild effusion(fluid accumulation) can be present.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Early phases of swelling are typically soft, although osteophyte production (bony outgrowths) may cause it to harden and become bony later on.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">5. Deformity:<\/span><\/h5>\n<p><span data-preserver-spaces=\"true\">Joints may appear distorted due to unusual bone remodeling and gradual joint degeneration.<\/span><\/p>\n<p><span data-preserver-spaces=\"true\">Common deformities include:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Knee OA: Genu Varum(bow-legged deformity) or Genu Valgum(Knock Knees).<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Hip OA: Leg shortening due to femoral head collapse.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Hand OA:<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\"> -Herbeden\u2019s nodes(bony swelling on DIP joints).<\/span><\/p>\n<p><span data-preserver-spaces=\"true\"> -Bouchard\u2019s nodes(bony swellings on the PIP joints).<\/span><\/p>\n<h5><span data-preserver-spaces=\"true\">6. Joint Instability and weakness:<\/span><\/h5>\n<p><span data-preserver-spaces=\"true\">As the disease progresses, the joint may become unstable leading t<\/span><span data-preserver-spaces=\"true\">o:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Giving way to the knee\/Knee buckling while walking.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Muscle weakness, especially in quadriceps and hip abductors.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Increased chances of falls due to difficulty in maintaining balance.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">7. Restricted Range of Motion(ROM):<\/span><\/h5>\n<p><span data-preserver-spaces=\"true\">The range of motion is gradually reduced due to pain, swelling, and structural changes,<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Early stage: Movement causes slight discomfort.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Last stage: Restricted movements, sometimes lead to joint contractures.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">8. Joint Tenderness:<\/span><\/h5>\n<ul>\n<li><span data-preserver-spaces=\"true\"> The patient presents pain while touching that area, especially along the joint line.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">When pressing the affected joints, localized pain is present.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">9. Functional impairment:<\/span><\/h5>\n<p><span data-preserver-spaces=\"true\">Patients experience difficulty in performing daily activities such as:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Climbing stairs.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Getting up from a chair.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Walking for long distances.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Gripping objects(in hand OA).<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Assistive devices like walking sticks are required in severe cases.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">10. Muscle Atrophy:<\/span><\/h5>\n<ul>\n<li><span data-preserver-spaces=\"true\">Further instability results from surrounding muscle weakening and atrophy brought on by joint disuse<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">In OA Knee it is commonly seen in the quadriceps(thigh muscles).<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">11.Synovitis(Intermittent inflammation):<\/span><\/h5>\n<p><span data-preserver-spaces=\"true\">In some cases, inflammatory flares occur with,<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Increased swelling.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Warmth over the joint.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Worsening pain.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Although it doesn&#8217;t have systemic symptoms, resembling rheumatoid arthritis.<\/span><\/li>\n<\/ul>\n<h1><span data-preserver-spaces=\"true\">DIAGNOSIS OF OSTEOARTHRITIS(OA):<\/span><\/h1>\n<p><span data-preserver-spaces=\"true\">Diagnosis of osteoarthritis (OA) involves a combination of clinical evaluation, imaging, and some laboratory tests.<\/span><\/p>\n<h4><span data-preserver-spaces=\"true\">1. CLINICAL ASSESSMENT:<\/span><\/h4>\n<p><span data-preserver-spaces=\"true\">Evaluate symptoms like:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Joint pain: That worsens with activity and improves with rest.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Stiffness, especially after an inactivity or in the morning.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">The affected joints have swelling and tenderness.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Crepitus(grating sound in the joint while moving).<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Restricted range of motion.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Deformities(in severe cases).<\/span><\/li>\n<\/ul>\n<h4><span data-preserver-spaces=\"true\">2. PHYSICAL EXAMINATION:<\/span><\/h4>\n<ul>\n<li><span data-preserver-spaces=\"true\">Check for joint tenderness, swelling, and range of motion.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Feel for crepitus or bone spurs.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Assess muscle strength and gait.<\/span><\/li>\n<\/ul>\n<h4><span data-preserver-spaces=\"true\">3.
