{"id":7942,"date":"2025-04-16T06:40:22","date_gmt":"2025-04-16T06:40:22","guid":{"rendered":"https:\/\/mauryaayurveda.com\/?p=7942"},"modified":"2025-04-16T06:40:22","modified_gmt":"2025-04-16T06:40:22","slug":"it-band-syndrome4-ayurvedic-treatments","status":"publish","type":"post","link":"https:\/\/mauryaayurveda.com\/ar\/it-band-syndrome4-ayurvedic-treatments\/","title":{"rendered":"IT BAND SYNDROME; FROM INFLAMMATION TO RESTORATION WITH AYURVEDA!"},"content":{"rendered":"<p>IT Band Syndrome(Iliotibial Band Syndrome\/ITBS) is an overuse injury characterized by pain on the outer(later)side of the knee, caused by inflammation or irritation of the iliotibial band(IT band) as it rubs against the lateral femoral epicondyle(bony prominence on the outer part of (femur\/thigh bone)during repetitive knee movements.<br \/>\nThe iliotibial band runs from the hip (iliac crest) to the lateral portion of the shinbone (tibia), just below the knee. It is a thick band of fascia (connective tissue) that runs along the outside of the thigh. It is formed when the fibers of two muscles unite.<\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Tensor fasciae latae(TFL).<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Gluteus maximus.<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">The IT band plays a major role in stabilizing the knee and hip during activities like walking, running, cycling, and climbing etc.<\/span><\/p>\n<h1><span data-preserver-spaces=\"true\">PATHOPHYSIOLOGY OF IT BAND SYNDROME(ITBS):<\/span><\/h1>\n<p><span data-preserver-spaces=\"true\">During repetitive activities; especially running or cycling-the knee continuously flexes and extends. As this happens:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">The IT band glides over the lateral femoral epicondyle.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Repetitive friction between the band and bone leads to inflammation, thickening, or tightness of the IT band.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">This causes pain and tenderness on the outer side of the knee, especially during the activity.<\/span><\/li>\n<\/ul>\n<h1><span data-preserver-spaces=\"true\">CLINICAL FEATURES OF IT BAND SYNDROME(ITBS):<\/span><\/h1>\n<h5><span data-preserver-spaces=\"true\">1.Lateral Knee Pain(Most Characteristic Feature):<\/span><\/h5>\n<ul>\n<li><span data-preserver-spaces=\"true\">Site: Pain is localized at the lateral femoral condyle(outer side of the knee).<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Nature of pain: Sharp or burning pain during activity, especially running, climbing stairs, or cycling.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Worsens with: Downhill running, prolonged sitting with knee bent, or after activity.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Relieved by: Rest and stopping the activity.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">2. Pain that increases with Activity:<\/span><\/h5>\n<ul>\n<li><span data-preserver-spaces=\"true\">Running downhill.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Climbing stairs.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Cycling.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Squatting.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Walking or running on uneven surfaces.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Pain often subsides with rest, but returns once activity resumes.<\/span><\/p>\n<h5><span data-preserver-spaces=\"true\">3. Pain with pressure:<\/span><\/h5>\n<ul>\n<li><span data-preserver-spaces=\"true\">Pain may worsen when pressing on the lateral side of the knee or lying on the affected side.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">4.Tenderness:<\/span><\/h5>\n<ul>\n<li><span data-preserver-spaces=\"true\">On palpation over the lateral epicondyle.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Sometimes, a snapping sensation may be felt when the IT band rubs over the condyle during knee flexion and extension.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">5. Swelling or Thickening:<\/span><\/h5>\n<ul>\n<li><span data-preserver-spaces=\"true\">Localized swelling over the lateral knee may be seen in more inflamed cases.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">It is not always visible but they feel like a thickened band on palpation.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">6.Tightness:<\/span><\/h5>\n<ul>\n<li><span data-preserver-spaces=\"true\">The IT band and tensor fascia lata(TFL) may feel tightened and shortened.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">7.Gait Changes:<\/span><\/h5>\n<ul>\n<li><span data-preserver-spaces=\"true\">In chronic cases, patients may alter their gait to reduce pain.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">It can lead to secondary issues in the hip or ankle due to compensatory mechanisms.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">8. Snapping or Popping sensation:<\/span><\/h5>\n<ul>\n<li><span data-preserver-spaces=\"true\">Snapping or popping sound or feeling as the knee bends and straightens, due to IT band rubbing over the bone.<\/span><\/li>\n<\/ul>\n<h1><span data-preserver-spaces=\"true\">CLINICAL TESTS FOR IT BAND SYNDROME(ITBS)<\/span><span data-preserver-spaces=\"true\">:<\/span><\/h1>\n<p><span data-preserver-spaces=\"true\">There are<\/span> <span data-preserver-spaces=\"true\">several<\/span><span data-preserver-spaces=\"true\"> physical examination tests used to assess ITBS.<\/span><\/p>\n<h5><span data-preserver-spaces=\"true\">1. Ober\u2019s Test:<\/span><\/h5>\n<p><img fetchpriority=\"high\" decoding=\"async\" class=\"alignnone wp-image-7945\" src=\"https:\/\/mauryaayurveda.com\/wp-content\/uploads\/2025\/04\/ober-test.gif\" alt=\"ober-test-IT BAND SYNDROME\" width=\"344\" height=\"508\" \/><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Purpose: To assess the tightness of the iliotibial band.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Procedure:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">The patient lies on their unaffected side.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">The examiner stands behind the patient and stabilizes the pelvis.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">The affected leg is flexed at the knee at 90 degrees and then abducted and extended at the hip.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">The leg is then allowed to drop toward the table passively.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Positive sign:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">The leg remains <\/span><span data-preserver-spaces=\"true\">in an abducted position<\/span><span data-preserver-spaces=\"true\"> and does not fall to the table, indicating a tight IT band.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">2. Noble\u2019s Compression Test:<\/span><\/h5>\n<p><img decoding=\"async\" class=\"alignnone wp-image-7946\" src=\"https:\/\/mauryaayurveda.com\/wp-content\/uploads\/2025\/04\/Noble-compression-test--300x300.png\" alt=\"Noble-compression-test-IT Band syndrome\" width=\"390\" height=\"390\" \/><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Purpose: To reproduce pain associated with ITBS and confirm inflammation over the lateral femoral epicondyle.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">Procedure:<\/span><\/h5>\n<ul>\n<li><span data-preserver-spaces=\"true\">The patient lies supine with the hip and knee flexed at 90 degrees.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">The examiner applies pressure to the lateral femoral epicondyle(about 1-2 cm above the lateral joint line).<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">The knee is then slowly extended while maintaining pressure.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Positive sign:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Pain over the lateral femoral epicondyle around 30 degrees of flexion during knee extension suggests ITBS.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">3. Renne\u2019s Test:<\/span><\/h5>\n<p><img decoding=\"async\" class=\"alignnone wp-image-7947\" src=\"https:\/\/mauryaayurveda.com\/wp-content\/uploads\/2025\/04\/RENNES-TEST-300x169.webp\" alt=\"RENNE'S TEST-it band syndrome\" width=\"431\" height=\"243\" \/><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Purpose: A variation of Noble\u2019s tes<\/span><span data-preserver-spaces=\"true\">t, u<\/span><span data-preserver-spaces=\"true\">sed to detect lateral knee pain from ITBS in weight bearing.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Procedure:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">The patient stands and the examiner applies pressure to the lateral femoral epicondyle.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">The patient then performs a single-leg squat or knee flexion to about 30-40 degrees.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Positive sign:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Pain or discomfort at the lateral knee, especially around 30 degrees, indicates ITBS.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">4. Modified Thomas Test:<\/span><\/h5>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-7948\" src=\"https:\/\/mauryaayurveda.com\/wp-content\/uploads\/2025\/04\/THOMAS-TEST-290x300.jpg\" alt=\"THOMAS TEST-IT Band Syndrome\" width=\"412\" height=\"426\" \/><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Purpose: To assess tightness in the band iliopsoas, rectus femoris, and IT band.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Procedure:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">The patient lies supine and pulls one knee to their chest.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">The examiner observes the opposite leg for lifting off the table, abduction, or external rotation.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Positive signs for ITBS:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">The opposite leg abducts or externally rotates, suggesting tightness of the IT band.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">5. Palpation and Functional Tests:<\/span><\/h5>\n<ul>\n<li><span data-preserver-spaces=\"true\">Palpation: Tenderness upon palpation over the lateral femoral epicondyle or along the IT band.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Functional Tests:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Pain is often reproduced with <\/span><span data-preserver-spaces=\"true\">activities such as<\/span><span data-preserver-spaces=\"true\"> downhill running, squatting, or going downstairs.<\/span><\/li>\n<\/ul>\n<h4><span data-preserver-spaces=\"true\">Additional Diagnostic Tools:<\/span><\/h4>\n<h5><span data-preserver-spaces=\"true\">a)MRI(Magnetic Resonance Imaging):<\/span><\/h5>\n<ul>\n<li><span data-preserver-spaces=\"true\">Thickened IT band.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Inflammation or fluid between the It band and lateral femoral epicondyle.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">It helps to rule out lateral meniscus tears or joint pathology.<\/span><\/li>\n<\/ul>\n<h5><span data-preserver-spaces=\"true\">b)X-Ray:<\/span><\/h5>\n<ul>\n<li><span data-preserver-spaces=\"true\">Usually Normal.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Done to rule out bony abnormalities.<\/span><\/li>\n<\/ul>\n<h1><span data-preserver-spaces=\"true\">DIFFERENTIAL DIAGNOSIS OF IT BAND SYNDROME(ITBS):<\/span><\/h1>\n<h4><span data-preserver-spaces=\"true\">1. Lateral Meniscus Tear:<\/span><\/h4>\n<p><span data-preserver-spaces=\"true\">Pain Location:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Lateral Joint line(deeper than ITBS).<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Key Symptoms:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Locking or catching of the knee.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Pain worsens with twisting or squatting.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">History of trauma or sudden twisting injury.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Clinical Tests:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">McMurray\u2019s test.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Thessaly test.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Imaging:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">MRI confirms the diagnosis.<\/span><\/li>\n<\/ul>\n<h4><span data-preserver-spaces=\"true\">2.Lateral Collateral Ligament(LCL)Injury:<\/span><\/h4>\n<p><span data-preserver-spaces=\"true\">Pain:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Over collateral ligament(slightly above or below the lateral joint line).<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Key Symptoms:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">History of varus(inward)stress or trauma to the medial knee.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Instability during side-to-side movements.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Localized swelling and tenderness.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Clinical Tests:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Varus stress positive.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Imaging:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">MRI is <\/span><span data-preserver-spaces=\"true\">useful<\/span><span data-preserver-spaces=\"true\"> to assess ligament integrity.<\/span><\/li>\n<\/ul>\n<h4><span data-preserver-spaces=\"true\">3. Popliteus Tendinopathy or Injury:<\/span><\/h4>\n<p><span data-preserver-spaces=\"true\">Pain:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">The posterolateral aspect of the knee(deeper than ITB).<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Key Symptoms:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Pain during downhill running or declaration.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Snapping or catching sensation.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Clinical Tests:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Restricted knee flexion.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">External tibial rotation causes pain.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Imaging:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">MRI shows inflammation or tears.<\/span><\/li>\n<\/ul>\n<h4><span data-preserver-spaces=\"true\">4. Biceps Femoris Tendinopathy:<\/span><\/h4>\n<p><span data-preserver-spaces=\"true\">Pain:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Posterolateral knee, near fibula head.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Key Symptoms:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Pain with resisted knee flexion.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Local tenderness at the fibular head.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Common in sprinting athletes.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Clinical Clues:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Tender insertion at the fibular head differentiates from ITBS.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Imaging:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Ultrasound or MRI may show tendon changes.<\/span><\/li>\n<\/ul>\n<h4><span data-preserver-spaces=\"true\">5.Patellofemoral Pain Syndrome(PFPS):<\/span><\/h4>\n<ul>\n<li><span data-preserver-spaces=\"true\">Pain Location: Anterior or peripatellar, which<\/span> <span data-preserver-spaces=\"true\">can refer to the lateral side.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Key Symptoms:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Pain worsens with prolonged sitting, stair climbing, or squatting.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">No<\/span><span data-preserver-spaces=\"true\"> specific point tenderness over ITB.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Clinical Tests:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Clarke\u2019s Test(positive).<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Imaging:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">An X-ray may show patellar <\/span><span data-preserver-spaces=\"true\">maltracking<\/span><span data-preserver-spaces=\"true\">.<\/span><\/li>\n<\/ul>\n<h4><span data-preserver-spaces=\"true\">6. Stress Fracture of Lateral Tibial Plateau or Femur:<\/span><\/h4>\n<ul>\n<li><span data-preserver-spaces=\"true\">Pain: Lateral proximal tibia or distal femur.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Key Symptoms:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Deep, aching pain worsens with weight bearing.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">It may occur in endurance athletes or due to overtraining.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Clinical Clues:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Pain is constant, not activity-specific like ITBS.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Imaging:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Bone<\/span><span data-preserver-spaces=\"true\"> scan or MRI essential(X-ray may be normal initially).<\/span><\/li>\n<\/ul>\n<h4><span data-preserver-spaces=\"true\">7. Superior Tibiofibular Joint Dysfunction:<\/span><\/h4>\n<p><span data-preserver-spaces=\"true\">Pain:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">The lateral side of the knee, near the fibula head.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Key Symptoms:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Pain with rotational knee movement.<\/span><\/li>\n<li><span data-preserver-spaces=\"true\">Clicking or locking sensation.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Clinical Clues:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">Palpation over the fibular head reproduces pain.<\/span><\/li>\n<\/ul>\n<p><span data-preserver-spaces=\"true\">Imaging:<\/span><\/p>\n<ul>\n<li><span data-preserver-spaces=\"true\">MRI or joint-specific imaging helps.<\/span><\/li>\n<\/ul>\n<h4><span data-preserver-spaces=\"true\">8.
