This condition mainly affects the peripheral nerves, particularly in the hands and feet, but may also involve the autonomic, motor, or cranial nerves, resulting in various sensory and functional difficulties.<\/p>\n<h1><span style=\"font-weight: 400;\">TYPES AND CLINICAL SYMPTOMS OF DIABETIC NEUROPATHY:<\/span><\/h1>\n<p><span style=\"font-weight: 400;\">There are four main types of diabetic neuropathy, each with distinct clinical symptoms:<\/span><\/p>\n<h4><span style=\"font-weight: 400;\">1 <strong>Peripheral Neuropathy(Distal Symmetric Polyneuropathy)<\/strong>:<\/span><\/h4>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">It is the most common type of diabetic neuropathy.<\/span><\/li>\n<\/ul>\n<h5><span style=\"font-weight: 400;\">Symptoms:<\/span><\/h5>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Pain: Burning, stabbing, electric shock-like pain, usually worse at night.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Tingling or numbness: Often begins in the toes and progresses upward(stocking-glove pattern).<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Loss of sensation: Inability to feel temperature, pain, or touch, increasing risk of foot injuries.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Muscle weakness: Especially in the feet and ankles.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Poor coordination or balance, especially in the dark.<\/span><\/li>\n<\/ul>\n<h5><span style=\"font-weight: 400;\">Complications:<\/span><\/h5>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Foot ulcers due to unnoticed injuries<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Charcot joint(progressive degeneration of a weight bearing joint)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Amputations in severe cases<\/span><\/li>\n<\/ul>\n<h4><span style=\"font-weight: 400;\">2 <strong>Autonomic Neuropathy:<\/strong><\/span><\/h4>\n<p><span style=\"font-weight: 400;\">Affects nerves that control involuntary functions, such as digestion, heart rate, and blood pressure.<\/span><\/p>\n<h5><span style=\"font-weight: 400;\">Symptoms:<\/span><\/h5>\n<p><span style=\"font-weight: 400;\">Cardiovascular:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Resting tachycardia(high resting heart rate)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Orthostatic hypotension(drop in BP when standing)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Exercise intolerance<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Gastrointestinal:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Gastroparesis(delayed stomach emptying, nausea, bloating, early satiety)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Constipation or diarrhoea<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Faecal incontinence<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Genitourinary:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Bladder dysfunction: Incomplete emptying, urinary retention<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Erectile dysfunction in men<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Vaginal dryness in women<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Sweat glands:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Decreased sweating(anhidrosis) in feet and legs.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Excessive sweating in the upper body or after meals(gustatory sweating).<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Pupillary involvement:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Poor adaptation to darkness.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Difficulty with night vision.<\/span><\/li>\n<\/ul>\n<h4><span style=\"font-weight: 400;\">3. <strong>Proximal Neuropathy(Diabetic Amyotrophy or Lumbosacral Radiculoplexus Neuropathy):<\/strong><\/span><\/h4>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">This type is less common and usually affects older adults with type 2 diabetes.<\/span><\/li>\n<\/ul>\n<h5><span style=\"font-weight: 400;\">Symptoms:<\/span><\/h5>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Sudden, severe pain in the hip, thigh or buttock (usually one side).<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Muscle weakness in the thigh or leg.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Difficulty rising from a sitting position or climbing stairs.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Weight loss(often rapid and significant).<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Reflex changes (eg: Absent knee jerk reflex).<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Usually improves over months to years, but can cause significant disability.<\/span><\/p>\n<h4><span style=\"font-weight: 400;\">4. <strong>Focal Neuropathy(Mononeuropathy):<\/strong><\/span><\/h4>\n<p><span style=\"font-weight: 400;\">Sudden weakness or pain in a single nerve or group of nerves, usually affecting the head, torso, or leg.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Cranial Nerve Involvement:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">CN III(Oculomotor Nerve): Sudden onset of double vision and drooping eyelid with preserved pupil reflex(pupil sparing).<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">CN VI(Abducens Nerve): Lateral gaze palsy diplopia<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Peripheral Involvement:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Pain in specific areas like the lower back, chest, abdomen, or front of thigh.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Wrist drop or foot drop(due to radial or peroneal nerve palsy)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Carpal tunnel syndrome(median nerve entrapment) is also common.<\/span><\/li>\n<\/ul>\n<h1><span style=\"font-weight: 400;\">DIAGNOSIS OF DIABETIC NEUROPATHY:<\/span><\/h1>\n<h4><span style=\"font-weight: 400;\">1 <strong>Clinical history and symptom analysis:<\/strong><\/span><\/h4>\n<ul>\n<li><span style=\"font-weight: 400;\">Do the clinical evaluation thoroughly:<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Sensory symptom: Numbness, tingling(pins and needles), burning, or pain, especially in feet and hands.<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Motor symptoms: Muscle weakness, imbalance or foot deformities.<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Autonomic symptoms: Dizziness, digestive issues, sexual dysfunction, bladder problems, or heart rate variability.<\/span><\/li>\n<li><span style=\"font-weight: 400;\">The pattern is often \u201cstocking gloves\u201d, starting from the toes and moving upward.<\/span><\/li>\n<\/ul>\n<h4><span style=\"font-weight: 400;\">2 <strong>Physical Examination:<\/strong><\/span><\/h4>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Reflexes(especially ankle reflexes)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Vibration sensation(using a tuning fork)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Pinprick and temperature sensation<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Position sense(proprioception)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Muscle strength and tone<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Foot inspection for ulcers, deformities, or calluses<\/span><\/li>\n<\/ul>\n<h4><span style=\"font-weight: 400;\">3. <strong>Diagnostic Tests:<\/strong><\/span><\/h4>\n<h5><span style=\"font-weight: 400;\">A. <strong>Blood Test:<\/strong><\/span><\/h5>\n<p><span style=\"font-weight: 400;\">To rule out or confirm other causes of neuropathy:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">HbA1c: Long-term blood sugar control<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Vitamin B12 Levels<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Thyroid Function (TSH)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Kidney and liver function tests<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Autoimmune or inflammatory markers(like CRP, ESR etc.)<\/span><\/li>\n<\/ul>\n<h5><span style=\"font-weight: 400;\">B. <strong>Neurological Tests:<\/strong><\/span><\/h5>\n<h5><span style=\"font-weight: 400;\">1. Monofilament Test:<\/span><\/h5>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">A 10 g Semmes-Weinstein monofilament is pressed on the foot.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">If the patient cannot feel the filament, it suggests loss of protective sensation and risk of ulcers.<\/span><\/li>\n<\/ul>\n<h5><span style=\"font-weight: 400;\">2<\/span><span style=\"font-weight: 400;\">. Vibration Prick test:<\/span><\/h5>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">A 128 Hz tuning fork is used.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Reduced perception indicates large fibre neuropathy.<\/span><\/li>\n<\/ul>\n<h5><span style=\"font-weight: 400;\">3<\/span><span style=\"font-weight: 400;\">. Thermal or pinprick test:<\/span><\/h5>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Tests small fibre function.<\/span><\/li>\n<\/ul>\n<h5><span style=\"font-weight: 400;\">4. <\/span><span style=\"font-weight: 400;\">Nerve Conduction Studies(NCS):<\/span><\/h5>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Measures speed and strength of signals in nerves.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Slowed conduction velocity or reduced amplitude indicates neuropathy.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Useful for larger fibre involvement.<\/span><\/li>\n<\/ul>\n<h5><span style=\"font-weight: 400;\">5. <\/span><span style=\"font-weight: 400;\">Electromyography(EMG):<\/span><\/h5>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Evaluates muscle electrical activity:<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Helps in differentiating between nerve and muscle disease.<\/span><\/li>\n<\/ul>\n<h5><span style=\"font-weight: 400;\">6. <\/span><span style=\"font-weight: 400;\">Quantitative sensory testing:<\/span><\/h5>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Measures threshold to vibration, heat, or cold.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Assess small and larger fibre involvement.<\/span><\/li>\n<\/ul>\n<h5><span style=\"font-weight: 400;\">C. <strong>Autonomic Testing:<\/strong><\/span><\/h5>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Heat rate variability(Ewing\u2019s tests)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Postural blood pressure changes(Orthostatic hypotension)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Sweat testing(QSART or thermoregulatory sweat test)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Gastrointestinal tests(eg: gastric emptying study)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Bladder function tests<\/span><\/li>\n<\/ul>\n<h4><span style=\"font-weight: 400;\">4. <strong>Diabetic Foot risk assessment:<\/strong><\/span><\/h4>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Checking for ulceration risk using monofilament and visual inspection.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Vascular assessment(peripheral pulses, doppler studies if needed).<\/span><\/li>\n<\/ul>\n<h1><span style=\"font-weight: 400;\">DIFFERENTIAL DIAGNOSIS OF DIABETIC NEUROPATHY:<\/span><\/h1>\n<h5><span style=\"font-weight: 400;\">1 <strong>Alcoholic Neuropathy:<\/strong><\/span><\/h5>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Cause: Chronic alcohol consumption.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Indication: History of alcohol abuse, nutritional deficiencies(B1, B6, B12).<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Symptoms: Similar to diabetic neuropathy, but with more prominent muscle wasting.<\/span><\/li>\n<\/ul>\n<h5><span style=\"font-weight: 400;\">2 <strong>Vitamin\u00a0 deficiency Neuropathy:<\/strong><\/span><\/h5>\n<p><span style=\"font-weight: 400;\">Vitamin B12 deficiency:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Indication: Anaemia, glossitis, cognitive changes<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Symptoms: Sensory Ataxia, paresthesia, positive Romberg sign<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Other vitamins: B1, B6, E<\/span><\/li>\n<\/ul>\n<h5><span style=\"font-weight: 400;\">3. <strong>Hypothyroid Neuropathy:<\/strong><\/span><\/h5>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Indication: Fatigue, weight gain, constipation, cold intolerance<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Neuropathy type: Often entrapment(like carpal tunnel) or distal polyneuropathy<\/span><\/li>\n<\/ul>\n<h5><span style=\"font-weight: 400;\">4<strong>. Uremic Neuropathy:<\/strong><\/span><\/h5>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Cause: Chronic kidney disease<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Indication: Elevated creatinine, GFR&lt;30, dialysis history<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Symptoms: Symmetric sensory motor neuropathy<\/span><\/li>\n<\/ul>\n<h5><span style=\"font-weight: 400;\">5. <strong>Chronic Inflammatory Demyelinating Polyneuropathy(CIDP):<\/strong><\/span><\/h5>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Indications: Progressive weakness over 8+ weeks, large-fiber involvement, areflexia<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">NCS\/EMG: Demyelination pattern<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Note: CIDP may be mistaken for diabetic neuropathy, but it is treatable with immunotherapy.<\/span><\/li>\n<\/ul>\n<h5><span style=\"font-weight: 400;\">6. <strong>Hereditary Neuropathies(eg: Charcot Marie tooth disease):<\/strong><\/span><\/h5>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Indications: Family history, high arches(pes cavus), hammer toes.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Onset: Often in adolescence or early adulthood.<\/span><\/li>\n<\/ul>\n<h5><span style=\"font-weight: 400;\">7. <strong>Toxic Neuropathy:<\/strong><\/span><\/h5>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Causes: Drugs(eg: Chemotherapy-vincristine, cisplatin), heavy metals(arsenic lead)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Indications: Exposure history, sudden onset after medication<\/span><\/li>\n<\/ul>\n<h5><span style=\"font-weight: 400;\">8. <strong>HIV or AIDS associated Neuropathy:<\/strong><\/span><\/h5>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Indication: HIV infection, immunosuppression<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Pattern: Distal symmetric polyneuropathy or inflammatory neuropathy<\/span><\/li>\n<\/ul>\n<h5><span style=\"font-weight: 400;\">9. <strong>Guillain Barre Syndrome(GBS):<\/strong><\/span><\/h5>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Examples: Carpal tunnel syndrome, tarsal tunnel syndrome<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Indication: Focal symptoms, weakness, or sensory loss in nerve distribution.<\/span><\/li>\n<\/ul>\n<h5><span style=\"font-weight: 400;\">10. <strong>Entrapment Neuropathies:<\/strong><\/span><\/h5>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Examples: Carpal tunnel syndrome, tarsal tunnel syndrome<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Indication: Focal symptoms, weakness or sensory loss in nerve distribution.<\/span><\/li>\n<\/ul>\n<h5><span style=\"font-weight: 400;\">11.
