Cerebral atrophy is the loss of cells of the brain which could be either in morphological presentation like shrinkage of the cells or in anatomical presentation like cell damage. In morphological presentation it is frequently seen as loss of volume of cells or shrinkage in computerized tomographic study.
It is a common end point of many of the diseases affecting the brain as well as the central nervous system. It is also seen in old age as a part of degeneration in many of the neuro degenerative diseases. All the imaging changes of the elderly are labeled as cerebral atrophy even when the patient has normal cognition.
It is akin to hydrocephalus as the brain parenchymal volume is reduced and there will be compensatory enlargement of the spaces due to CSF. This filling of the CSF is rather gradual than that of a pathological insult like a hemorrhage which occurs after an RTA where also a focal volume difference occurs.
It can’t be identified as a uniform disease entity, so the mode of presentation also varies. The diagnosis is often incidental when the imaging studies are done for some other diseases.
Cognitive dysfunction and acute confusion are common symptoms for which the patients with atrophy usually undergo imaging.
Following are the underlying pathological causes that can be broadly distinguished based on whether the atrophy is focal or generalized
- cerebrovascular disease
- end-stage multiple sclerosis
- alcohol abuse (cerebellar vermian atrophy)
- drug abuse
- certain neurodegenerative diseases
- post-ischemic injury
- post-traumatic (hemorrhage, contusion)
- certain neurogenic disorders
CT and MRI are done mainly to diagnose the condition, but MRI is more precise and sensitive in detecting even the mild focal lesions and the atrophic changes in the nuclei.

Atrophy is degeneration or loss of normal tonicity of cells which is always a part of aging. But apart from aging it is seen in some neuro degenerative diseases affecting the central nervous system. All these are categorised as vata vyadhi in ayurveda.
So the treatment line revolves around the vata vyadhi principle where treatment is based on unctuous substances.
Vasthi is regarded as ardha chikitsa in ayurveda as it is very beneficial in neurological conditions. It promotes longevity and retards ageing. Vasthi in some other forms like raja yapana vasthi, sneha vasthi are very beneficial in such conditions.
